Your First Visit
Depending on the purpose of your visit, you should expect your initial consultation with Dr. Judy to take about 2 hours.
During this initial visit, you and Dr. Judy will discuss your overall health, your lifestyle, stress management and nutrition, as well as your past medical history, current health concerns, health goals, and any medications or supplementation that you might be currently taking.
A comprehensive physical exam will also be completed during this appointment and additional testing may be ordered.
At the conclusion of the visit Dr. Judy will make her treatment recommendations.
Please plan to arrive 15 minutes prior to your appointment and bring your intake form with you. Any additional information you can bring regarding your past medical history and current treatments is also helpful.

When completing the form, please answer the questions carefully, thoughtfully, and accurately. Many of the questions may not seem directly related to your problem or main complaint, however, the information you provide will help us to begin to understand your current state of health and determine which treatments or therapies are best suited for you.

Billing and Insurance Information
In the state of Arizona, Naturopathic Medical Doctors are not covered by insurance at the time of service. Rather, we will provide you with billing sheets, which have diagnosis codes and procedure codes, for you to submit directly to your insurance company for reimbursement. It is up to your insurance company to decide whether to reimburse you for your visit and to select the amount they are willing to cover.
However, we do accept health savings and flex spending accounts to cover all of your treatment services and supplement purchases at our office, and we process your insurance for lab work done in-house. We can also provide you with a lab order to have your blood drawn at an outside lab (Sonora Quest or LabCorp) if necessary.
